Attach and include Intake Forms, Consent Forms & Questions
Maybe a way to personalize questions or forms for client to fill out prior to booking an appointment. There are certain services offered that require the client to be 18, fill out certain information, or read over cancellation/appointment policy. It would be great to be able to personalize and incorporate that into the online booking system before they can place an appointment.
We do plan on adding custom intake forms to Shedul so that you can manage everything under one roof.
In the meantime, many businesses are using Google Forms or TypeForm to create forms and questionnaires and adding the URL link to Shedul’s client Email Notifications.
You can edit notifications under "Setup">"Client Notifications" where you’ll see the different message types for Reminders, Confirmations, Reschedules, Cancellations and Thanks Yous.

Re: Intake/Medical History/Liability Forms
For those of us in a service industry that requires a medical history/intake form (ie. Esthetics, massage therapy, etc.) and/or liability form, it would be wonderful if we could incorporate a customizable medical intake form into the online booking and/or “create new client profile” pages (or skip and fill in at first appointment, if necessary), including client medical history and any other preferences the client may have for services that is directly available in the client profile, without having to link an external form, etc. This should be a separate tab within the clients’ profile labeled “Forms”, or “Files” (where we may also be able to attach photos, etc. for future reference and documentation of services/progress).
If this could be an interactive checklist (ie “check all that apply: Arthritis __, High Blood pressure __ “ etc.) that can be completed via the app, and upon completion and digital signature, automatically saved to their client profile/client card, that would be ideal. It would be an excellent way to organize and reference important forms such as this. Thanks!

Adding a feature to clients info/profiles to give us providers a chance to scan and add a document to the clients profile (signed consents, product recommendation forms, etc)

Client profile to link with there details and etc like medical history etc or even be able to scan a client card on to computer to import into documents file as a tab, and any other details need to link client together with documents.
would absoulty LOVE this, even make up different client cards (like in the reports, to selcts what service and appropriate client card/ concent) to link with patient for a consult :) pleaseeeee X
When a new client books online with us, they must sign a pre-authorization form with credit card information.
The form will also include a cancellation fee, to prevent no-shows and last minute cancellations.
Also somewhere for them to sign at the end of the form?
Being able to do up customisable forms for clients to fill out upon appointments for different services and client records also.
GDPR is coming quickly. When will we have an audit trail and consultation forms on shedul?
We would like to have a PDF client consent form for specific services that the Health Department requires for tattoos (cosmetic or otherwise / piercings).
If we could customize or upload the form and be able to print the current client information automatically on the from in the client area/or calendar that would be AMAZING! Also if we could have an area to upload and store ID's.
Any tips on how to do these things under two roofs? I want the patient health history to show up with their "client" on shedul, I want to be able to edit that health history, and I want to upload pictures of their signed consent forms as an image, attached to the health history
Would also be interested in having there be an intake form that could be drawn on, like on a front and back images of a body, such as used in massage therapy or medical offices.
We need a paperless consultation form where client can originally fill in customised qn’s to sign off on medical & skincare history on each appointment. It would need access on a Salon iPad that the client cannot also access any other sensitive business info.
Offline calendar view option for staff incase of connection problems. If for any reason the internet is down we want to still be able to view appointments booked at present.
Digital consultation forms that can be populated with client data and get e signatures at each appointment.
I 100% agree! With other scheduling software I'm able to add my evaluation form to the client profile.

I had this with Acuity and it was the best. We could ask them pre-appt questions so I would have an idea of what type of hairstyle they were looking for because they could attach pictures. Also I could set up a series of question to be different for each service or consultation.
